If your child wants to play sports at school this year, he/she will need a physical to participate. There are a few places around town, in addition to your family practitioner or pediatrician, where your child’s health can be assessed, and you can be provided a release form for your school’s athletic department. Some of the sites listed below also provide year-round support for sports injuries, rehabilitation and training. See individual websites for details.
